What to enter
Reference the identity/ownership, sanctions and relevant evidence checks, with the date. Retain the underlying review record rather than copying sensitive details into the form.

- Confirm uncertain or conflicting evidence through the relevant issuer or authority rather than treating a missing search result as a final decision.

12.2Decision and conditions
What to enter
Choose Registered, Registered – conditions recorded, or Not registered. Record outstanding actions and activities that need an addendum. A condition may block the affected tender disclosure, award, work, access or transfer.
12.3Reviewer and review trigger
What to enter
Record the reviewer’s name, role and date, and the next review date or trigger. Material changes and relevant expiry dates can require updated evidence without a fresh agreement.
